Insurance Information

If you do not have medical insurance and need help applying for it, please contact our Family Resource Counselors for help.

Cheshire Medical Center providers participate in a wide variety of managed care plans. Find a list of insurance companies we accept.

Ask your insurance company what your plan covers

Each medical insurance plan is different, so please call your insurance company to carefully review your health insurance coverage before you use our services. It is each patient’s responsibility to know and abide by the terms of their own insurance policy, and Cheshire Medical Center staff can’t know the details of your insurance plan.

Many insurance plans require referrals and precertifications for most services beyond regular physical exams with your primary care team, so it is important to call and ask. You will usually be responsible for paying an “out-of-pocket” cost for major procedures, even if you have medical insurance.

Contacting your insurance company

Each company has a member services department waiting to take your call and give you the information you need about your insurance plan. Find the phone number and your plan number on your medical card. If you have lost your card, contact your insurance provider at the number below, or on their website.

Phone numbers for some of the most common insurance plans are listed below:

  • Medicare A: 800-MEDICARE
  • Anthem NH BC/BS*: 800-225-2666
  • Cigna NH: 800-291-2466
  • NH Medicaid: 800-852-3345
  • VT Medicaid: 800-250-8427
