Ask your insurance company what your plan covers
Each medical insurance plan is different, so please call your insurance company to carefully review your health insurance coverage before you use our services. It is each patient’s responsibility to know and abide by the terms of their own insurance policy, and Cheshire Medical Center staff can’t know the details of your insurance plan.
Many insurance plans require referrals and precertifications for most services beyond regular physical exams with your primary care team, so it is important to call and ask. You will usually be responsible for paying an “out-of-pocket” cost for major procedures, even if you have medical insurance.
Contacting your insurance company
Each company has a member services department waiting to take your call and give you the information you need about your insurance plan. Find the phone number and your plan number on your medical card. If you have lost your card, contact your insurance provider at the number below, or on their website.

New Hampshire Medicaid
If you receive benefits under the Aid to Families with Dependent Children (AFDC) program, contact the state Community Health Aide Program (CHAP) office to find out about programs offered by Tufts.

Cheshire Medical Center is participating in two available New Hampshire Medicaid health plans. The available plans are New Hampshire Healthy Families and Well Sense. For more information about the individual plans, please visit the New Hampshire Department of Health and Human Services website.
Health insurance marketplace
The Open Enrollment period for the Health Insurance Marketplace is November 1 to December 15 for coverage beginning on January 1. Individuals may also qualify for Special Enrollment Periods outside of Open Enrollment, if they experience a qualifying life event.
