New patients
Thank you for choosing our care team for your medical care. If you are a new patient, please ask your primary care provider (PCP) or other provider to forward a referral with all pertinent medical records. If you do not have a PCP or referring doctor, please call our appointment line to talk with our team. Please see our referrals and precertification page for more information about referrals.
Once we receive your referral, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary will explain your appointments to you in detail.
What to bring to your appointment
If this is your first visit to Cheshire Medical Center’s Cardiology Department, please bring the following with you:
- Previous cardiology records
- Medications and/or medication list
- Medical history form
Returning patients
If you are a returning patient, you may contact your provider securely through the myDH patient portal and the MyChart app. Use myDH to request an appointment or communicate with your health care team. As a returning patient, you can also call our department's appointment phone numbers.
What clothing to wear for your appointment
Because the provider may need to perform an electrocardiogram (EKG) during your cardiology visit, we suggest you wear a two-piece outfit such as slacks and a shirt/blouse for your comfort and convenience.

Please forward all related medical records, including echocardiogram if results are available.
