New patients
Thank you for choosing our care team for your medical care. If you are a new patient, your insurance provider may require a referral or precertification to cover our services. If so, please ask your primary care provider (PCP) or referring doctor to forward a referral to us with all pertinent medical records. If you do not have a PCP or referring doctor, or your insurance company does not require a referral, please call our appointment line to talk with our team directly.
We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary explains your appointments to you in detail.
Returning patients
If you are a returning patient, you may contact your provider through the myD-H patient portal and the MyChart app. Use myD-H to request an appointment or communicate with your health care team, and more. As a returning patient, you can also call our department's appointment phone numbers.
