Surgical procedures are performed by many departments
Please contact the department or specialty performing your surgery directly for more information. Minor surgeries are often perfomed within a specialty department, such as minor eye surgeries in Ophthalmology or skin cancer surgeries in Dermatology. Otherwise, Cheshire's Operating Rooms are shared between all surgical departments and the department responsible for your surgery is responsible for scheduling your care.
New patients
Thank you for choosing Cheshire Medical Center for your procedure. If you are a new patient, please ask your primary care provider (PCP) or referring provider to forward all pertinent medical records to the department responsible for your assessment and surgery.
If you do not have a PCP or referring doctor, please call the appropriate specialty department directly to talk about the next steps in your care. Please see our referrals and precertifications page for more information about referrals.
Once the team performing your surgery has your referral, they will contact you to schedule your appointment. Their scheduling secretary explains your appointments to you in detail.
Use myD-H to contact your providers
You may contact your providers securely through the myD-H patient portal and the MyChart app. Use myD-H to request an appointment, communicate with your healthcare team, and more.
