New patients
Thank you for choosing our care team for your medical care. If you are a new patient to the Urology department, please check with your insurance company to find out if you need a referral to cover urology appointments. Please see our referrals and precertifications page for more information about referrals.
Please ask your primary care provider (PCP) or other provider to forward all relevant medical records to us. If you do not have a referring provider, please call our appointment line to talk with our team.
Once we receive a referral from your PCP, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary will explain your appointments to you in detail.
Returning patients
If you are a returning patient, you may contact your provider securely through the myDH patient portal and the MyChart app. Use myDH to request an appointment or communicate with your health care team, and more. As a returning patient, you can also call our department's appointment phone numbers.
